
1st Department of Surgery
Establishment / Operation / Installation:
The 1st University Surgical Clinic was established by Presidential Decree (P.D.) 459/1987 (A’ 215) (Government Gazette 215/8-12-1987) and was installed at the University General Hospital of Alexandroupolis by the joint ministerial decision Υ4α /29261/03 of the Ministers of Health, Welfare and Social Security, and National Education and Religious Affairs (Government Gazette 750/11-6-2003). Originally, the 1st University Surgical Clinic was established by Law 641/1977 (Government Gazette A’ 200/77) as a branch of the Aristotle University of Thessaloniki and was transferred to DUTH by Article 70 par. 20 of Law 1566/85 as a Surgical Clinic, assigned to the Department of Surgery. It was renamed the “1st University Surgical Clinic” by ministerial decision B1/349/22.08.1996 (Government Gazette B’ 792/1996) and was installed at the University General Hospital of Alexandroupolis (UGHA) by the joint ministerial decision Υ4α/29261/2003 (Government Gazette 750/11.06.2003).
The Clinic is located on the 1st floor of Building 3 of the UGHA. It currently operates with 22 beds, although it is situated in a 34-bed Nursing Unit of the hospital, due to co-location in the same nursing wing (“K. I. Manolas” wing) with the University ENT Clinic of DUTH. Following decision 35th /26.11.2018 of the Regular Meeting of the Board of Directors of the unified Legal Entity of Public Law “University General Hospital of Evros” regarding the suspension of the National Health System (ESY) Surgical Department, an additional 7 (seven) beds for surgical patients were allocated to each of the two University Clinics (1st and 2nd Surgical Clinics).

Mission / Purpose:
The purpose of the 1st University Surgical Clinic is:
a) To meet the educational and research needs of the DUTH Department of Medicine, at both undergraduate and postgraduate levels, in subjects falling within the field and activities of a Surgical Clinic, as defined in Article 1 of the Clinic’s Internal Regulations.
b) Scientific and clinical activity concerning the prevention, diagnosis, surgery, as well as conservative treatment and rehabilitation of surgical diseases and trauma on both an emergency and elective basis.
c) The training and teaching of Medical students across three years of study, the training and teaching of Resident Physicians in surgical specialties—particularly General Surgery—as well as modern research/academic work.
d) The conduct of doctoral and postdoctoral research in the clinic’s scientific field in accordance with current legislation, as well as research work following the University’s (DUTH) Internal Regulations, the Medical Department’s Curriculum, and the legislative provisions of the Ministry of Education and the Ministry of Health.
e) Collaboration and exchange of scientific knowledge with other academic institutions, both domestic and international, as well as the organization of scientific lectures, conferences, symposia, seminars, and practical workshops aimed at promoting knowledge and updates on the Clinic’s subject matter.
f) Cooperation with public sector bodies, local government, and international scientific and social organizations that contribute to the study and application of proposals and alternative methods for managing surgical diseases.
g) Informing and raising awareness within the community regarding the prevention and therapeutic management of surgical diseases.
h) The preparation and publication of brochures, monographs, and books on surgical topics.
As a University Clinic, it is mandated to produce work in three directions: education, research, and clinical Surgery.
Education concerns:
a) Students of the Department of Medicine of the School of Health Sciences of DUTH.
b) Residents in General Surgery working toward their specialty certification.
c) Residents of all other Surgical specialties.
d) Residents in General Practice (Family Medicine).
e) Rural Doctors (according to the provisions).
f) Specialized Surgeons and Specialists in other Surgical fields.
g) Specialized Clinical Scientists or other Health Profession Scientists (ETEP, EDIP, etc.).
h) Scientists of other specialties related to the subject areas of the Department of Medicine.
i) Nursing Students.
j) Nursing staff of the University General Hospital of Alexandroupolis (UGHA) or other nursing facilities.
Student education is both theoretical and practical, conducted by Faculty members (DEP), health professionals of the Clinic, or other recognized scientists in accordance with current regulations and the DUTH Internal Regulations, under the coordination and responsibility of the Clinic Director. Theoretical education includes attendance of courses, lectures, seminars, tutorials, etc., according to the Curriculum, the organization of educational work—as defined by MODIP (Quality Assurance Unit)—and the Clinic’s Educational Program. Practical education includes training students in the field of Surgery, as well as familiarization and application of medical procedures and techniques on patients hospitalized in the clinic or treated at the Emergency Department (TEP) or Outpatient Clinics (TEI), under the supervision and responsibility of the Clinic’s Faculty members.
The training of Resident Physicians is also theoretical and practical, provided by the Clinic’s Faculty members and other Scientific associates, under the coordination and responsibility of the Clinic Director. Theoretical training includes courses across all fields of Surgery, lectures, practical workshops, presentation and discussion of interesting clinical cases, and literature updates. Furthermore, the medical and nursing staff of the clinic is required to participate in and attend seminars, conferences, symposia, and other scientific events organized by the clinic or other clinics and scientific societies. Practical training includes the evaluation, monitoring, and treatment of the clinic’s patients, as well as participation in surgical operations and other invasive procedures and techniques.
The training of fellows in general surgery subspecialties, postdoctoral researchers, postgraduate students, doctoral candidates, academic fellows, or other health scientists in Surgery is also theoretical and practical. It is conducted by the Clinic’s Faculty members under the coordination and responsibility of the Clinic Director. Theoretical training includes courses covering the predefined required syllabus, tutorials, literature updates, and the presentation and discussion of clinical cases. Practical training includes participation in all practical, invasive, or minimally invasive treatment methods and techniques, the implementation of new diagnostic and therapeutic methods, and the acquisition of experience in the respective field.
Research involves the design, conduct, and publication of clinical and experimental studies by Faculty members and other medical staff of the Clinic, either independently or in collaboration with other Faculty members, physicians, or researchers from other clinics or scientific and academic bodies of the same hospital or other hospitals and departments of the same or other universities, both domestic and international. Research is a primary concern of the clinic and is carried out under the supervision of the Director and the Faculty members.
Research and authorship are also goals of a University Clinic and serve as the scientific means for the promotion and evolution of medical practice.
Clinical work includes:
a) Clinical diagnosis, diagnostic investigation, therapeutic surgical or conservative management, care, and medical hospitalization of patients with surgical diseases or trauma, who are hospitalized in the Clinic and its units or treated in its outpatient clinics.
b) Participation of the Clinic in the on-call program of the University General Hospital of Alexandroupolis (UGHA) for the admission, investigation, and management of patients with surgical diseases or trauma under emergency and urgent healthcare service conditions.
The Faculty members and National Health System (ESY) physicians of the Clinic, accompanied by resident physicians, students, the Head Nurse, and the Clinic’s nursing staff, visit patients for clinical evaluation and monitoring, determination of nursing care and treatment plans, supervision, and therapy during daily medical rounds, as well as during educational rounds where medical diagnosis and treatment issues are analyzed theoretically.
Activities:
The Clinic’s staff provides medical services to outpatients and inpatients, covering specialized clinics, units, regular outpatient clinics, the Emergency Department, the Nursing Ward, scheduled and emergency surgeries, as well as day surgeries (Septic Operating Room). In addition to its clinical and academic work, the Clinic places particular emphasis on the training of resident surgeons, physicians, specialized scientists, and medical students.

Breast and Breast Surgery Unit

The “Breast and Breast Surgery Unit” specializes in surgical diseases of the breast and breast cancer surgery for both sexes, across all stages of: screening, diagnostic and therapeutic planning, surgical oncology and oncoplastic breast surgery, plastic breast reconstruction, coordination of recovery and post-operative follow-up. It also focuses on multidisciplinary collaboration within the framework of medical and radiation oncology treatment, psycho-social support, and the molecular/genetic study and identification of hereditary breast cancer and genetic counseling. Furthermore, it covers the surgical pathology of the male breast (gynecomastia, mastodynia, male breast cancer, hereditary male breast cancer). Upon its establishment, the Unit will additionally serve at a postgraduate level the clinical and research work falling within the field of breast surgery.
Unit of Metabolic and Nutritional Disorders in Surgical Patients

The “Unit of Metabolic and Nutritional Disorders in Surgical Patients” focuses specifically on the study of nutritional pathology, the indications and methods of nutritional support, and the management of severe nutritional disorders and abnormal BMI in surgical patients and clinical surgical practice. This includes scientific data regarding the selection or modification of various total parenteral nutrition solutions (where on-site preparation in a specialized lab 24-48h prior to administration is no longer required), as well as semi-elemental or elemental parenteral and enteral nutrition. The Unit also focuses on new nutritional support techniques, the selection of artificial nutrition in outpatient settings—particularly for oncology patients—and perioperative support in obesity surgery, both in-hospital and outpatient.
Surgical Intestinal Diseases Unit

The “Surgical Intestinal Diseases Unit” specializes in the study of surgical diseases and operations of the intestine, aligned with modern knowledge of the pathophysiology of underlying pro-inflammatory or inflammatory and neoplastic processes at the cellular level. It also focuses on alterations of the intestinal microbiome in all surgical diseases of the gastrointestinal tract, including idiopathic diseases, which have modified contemporary therapeutics and the indications, techniques, and types of open and/or laparoscopic and/or assisted surgical procedures.
Breast Diseases Clinic

The specialized “Breast Surgery and Breast Surgical Oncology Clinic” focuses on breast diseases and modern breast diagnostics and surgery. The Scientific Director is the Professor of Surgery at DUTH, Eleni Efremidou, who possesses the respective scientific postgraduate training in recognized domestic and international centers, as well as the corresponding clinical experience.
UNIVERSITY STAFF
The 1st University Surgical Clinic of DUTH is staffed by members and personnel falling under the categories defined by Article 50 of Law 4957 (Government Gazette 141/21.07.2022, Issue A’).
Specifically, the Clinic, its Units, and its Outpatient Clinics may employ:
a) Faculty Members (D.E.P.), Special Education Staff (E.E.P.), Laboratory Teaching Staff (E.DI.P.), and Special Technical Laboratory Staff (E.T.E.P.) of DUTH, or other domestic and international Higher Education Institutions (HEIs) with a scientific field related to the educational, research, and clinical activity of the Clinic.
b) Physicians (Specialists, Residents) and medical personnel of the National Health System (E.S.Y.) who are appointed to or serve at the University General Hospital of Alexandroupolis (UGHA) in accordance with current regulations.
c) Specialized scientists engaged in undergraduate, postgraduate, postdoctoral, or doctoral work, according to the respective decisions.
d) Research, scientific, administrative, technical, and other personnel through programs/projects of the Special Account for Research Funds (E.L.K.E.) of DUTH or other Academic Institutions or Institutes.
e) Scientific Associates based on Article 58 of Law 4957 (Government Gazette 141/21.07.2022, Issue A’).
f) Academic Fellows or Adjunct Lecturers.
g) Professors Emeritus and retired professors based on Article 170 of Law 4957 (Government Gazette 141/21.07.2022, Issue A’).
Administration – Responsibilities
The 1st University Surgical Clinic of DUTH is administered by the Director, who is elected and must be a full-time member of the Faculty (DEP), holding the rank of Professor or Associate Professor. The term of office is three years. The re-election of the same person to the office of University Clinic Director is permitted without any limit on the number of terms. Faculty members (DEP) who are appointed to and serve in the 1st University Surgical Clinic and share the same scientific field as the Clinic have the right to submit a candidacy for the position of Director (Gov. Gazette 141/ 21.07.2022, Issue A’).
The Director of the 1st University Surgical Clinic holds the responsibilities defined in Article 55, par. 9 of Law 4957 (Gov. Gazette 141/ 21.07.2022, Issue A’), including, among others:
a) Heads and supervises the operation of the university Clinic.
b) Ensures the implementation of the Clinic’s internal operating regulations, the internal regulations of the Department and DUTH and their decisions, as well as compliance with current state legislation.
c) Acts as the Scientific Director for projects/programs of the Special Account for Research Funds (ELKE), which manages the university Clinic’s own resources.
d) Recommends to the Rector the appointment or assignment of university administrative staff to support the Clinic.
e) Exercises every other responsibility defined in the University’s internal regulations and the Clinic’s internal operating regulations.
Title – Seal
The 1st University Surgical Clinic bears the title “1st UNIVERSITY SURGICAL CLINIC OF THE DEPARTMENT OF MEDICINE OF THE DEMOCRITUS UNIVERSITY OF THRACE”. This title, along with the Director’s details, is inscribed on all the Clinic’s forms and documents, as well as on the signs within the Clinic’s premises.
For its correspondence, the Clinic uses a round seal, which bears the inscription “DEMOCRITUS UNIVERSITY OF THRACE ALEXANDROUPOLIS” on the outer circle and “DEPARTMENT OF MEDICINE 1st UNIVERSITY SURGICAL CLINIC” on the inner circle.
For international correspondence, there is a corresponding seal in English, which the Clinic uses for its foreign-language communications.

EDUCATION – UNDERGRADUATE LEVEL
Within the Undergraduate Program of Studies of the DUTH Department of Medicine, the 1st University Surgical Clinic is one of the department’s two University Surgical Clinics. It is tasked with the educational work for the Department’s medical students and is responsible for the theoretical teaching of the scientific field of the Surgical Medical Specialty, as well as the practical-clinical training, familiarization, and experience in techniques, methods, technology, and their application to surgical diseases, surgical complications, and trauma. This spans four academic semesters, corresponding to three years of study and three mandatory courses:
- Physical Signs in Clinical Surgery / taught in the 6th semester (Spring)
- Surgical Pathology / taught in the 7th semester (Winter)
- Surgery / Clinical Training / 11th and 12th semesters (Winter – Spring)
These courses are taught in accordance with the Department’s Curriculum and the Clinic’s Educational Program, featuring theoretical lectures across the entire field of Surgery. These lectures are held in amphitheaters with a joint program coordinated by both University Surgical Clinics. Additionally, the education and teaching include clinical training in the hospital wards with patients, following a separate schedule for each of the two University Surgical Clinics with equal training time. The Educational Program of the 1st University Surgical Clinic includes tutorial exercises, clinical skills practicals, and participation/presence in surgical operations and interventions.

Students sign the attendance sheet twice every day, once upon arrival and once upon departure.
DAILY ATTENDANCE: In the Clinic’s wards, Outpatient Clinics (TEI), and the Emergency Department (TEP), in collaboration with resident and specialist physicians. Attendance of surgical operations.
ON-CALL SHIFTS: Students attend both External and Internal On-call shifts; the number of shifts on holidays and weekends (Sat-Sun) will be equally distributed among students. Upon completion of the shift, an attendance certification is signed on each student’s On-call Log by the first assistant on duty.
SCHEDULED SURGERIES:
Accompanying the patient to the operating room as the attending physician, and observation of / participation in the surgical procedure and recovery process, as well as accompanying the patient upon their return to the Clinic.
ΤHESIS / FINAL PROJECT:
The preparation of a project in the scientific field of Surgery, conducted in groups of 3-4 people under the supervision and guidance of a Faculty member and a resident physician.
During the last two weeks of the three-month training period, the projects are presented to fellow students and the Clinic’s medical staff. These projects are graded based on their completeness and the approach and presentation of the topic.

SAFETY AND HYGIENE RULES

RULES OF CONDUCT FOR MEDICAL STUDENTS DURING CLINICAL TRAINING:
The engagement of students in the Clinic on a daily, long-term basis and their regular contact and communication with patients, physicians, and paramedical staff makes the adoption of specific rules of conduct and appearance imperative for students.
For this reason, it is considered necessary to present basic principles of ethics and behavior, primarily towards patients. The points highlighted below are related to the subjects taught in the training cycles, ensuring that students are purposefully informed.
More specifically:
Introduce yourselves. In every contact with patients, you must explain that you are students, state your name, and the reason you are there. This is always essential for building a relationship of trust and honesty with the patient. The same practice should also be followed in any communication with the medical and paramedical staff.
External appearance is a sign of respect towards both the patient and your professional status. Your appearance must be neat and clean, including the use of a medical lab coat, for the good condition of which you are responsible. Hair and nails must be well-groomed and clean. The use of chewing gum, food, or drink during a patient examination is considered unacceptable and constitutes behavior that lacks respect toward the patient. Clothing and footwear must also be appropriate.
At this stage of your training, you will be under supervision within the Hospital premises. This fact alone protects you and exempts you from formal or time-consuming procedures. Nevertheless, never examine a patient without the presence of a third person (e.g., the patient’s companion or a member of the nursing staff). Furthermore, asking for permission or consent from the medical and paramedical staff to observe or participate in providing medical care to patients is always necessary.
If at any point during your communication with the patient (history taking or clinical examination) you feel that communication is becoming difficult and the patient does not wish to continue speaking with you, do not persist and ask your supervising clinician for assistance. Never engage in a confrontation with the patient or their companions.
Your position and conduct during your presence at the Hospital must be professional and discreet. Overly warm or familiar relationships with patients or medical and nursing staff are not appropriate for medical students. Additionally, making comments about a patient’s condition or the patient in general creates a negative impression. Polite and formal behavior toward both the patient and the medical and nursing staff is another ‘skill’ that you must acquire.
The educational process and the framework of patient communication include explaining a diagnostic test or a therapeutic approach to the patient—a process you must learn and become familiar with.
During the clinical examination process, show respect for the patient’s dignity. Ensure there is a partition to isolate the patient and provide them with space for the process of undressing and dressing before and after the clinical examination.
Give clear instructions regarding the part of the body you will examine and which needs to be uncovered. Do not ask the patient to remove more clothing than necessary, and also explain that a clinical examination cannot be performed without the removal of garments. Discretion at this point must be constantly on your mind. Use a cover for parts of the body that have been uncovered but are not being examined at that moment, so the patient feels more comfortable.
Ensure that the paper on the examination table is present and clean.
